This is an exciting opportunity to join a dynamic team, supporting the planning and transformation of primary and community care services across the Fylde Coast. As a Transformation and Planning Officer the post holder will work as part of a dynamic team in delivering an effective service supporting the development of Primary Care Networks and Neighbourhoods and the wider planning and transformation of integrated services including end to end pathway re-design.

This is an ideal opportunity for those wishing to progress within a management role. Secondments will be considered.

Your role will be to provide support and oversight to the team, to ensure efficient and effective day to day operations. This will include informing the targeting of resources, and the development, implementation, monitoring and evaluation of tasks and projects being delivered. Complex information gathering and analysis, communications and stakeholder management are core to the role. You will also have responsibility for key deliverables aligned to the national Primary Care Network integration agenda.

You must have excellent organisation and management skills, experience of building effective working relationships with a wide range of stakeholders (including non-NHS) and a working understanding of service transformation projects.
